LIABILITY

I understand there is an inherent risk associated with any exercise program including voluntary participation in yoga, dance, meditation or movement classes that may result in injury.

I understand that I or my child may receive physical assists or adjustments to enhance or correct body posture during class by either the teacher or an assistant in class. I or my child will alert the teacher and assistant of any injury or impairment in advance before class begins or if I or my child do not want to receive any assists.

I acknowledge that I or my child have either had a physical examination and /or have been given permission from my physician to participate in an exercise program or that I or my child has decided to participate in an exercise program voluntarily and without the approval of my physician and do hereby assume all responsibility for mine or my child's participation in any movement activity associated with The Movement Loft, LLC.

I, my heirs, or legal representatives, do hereby waive and release The Movement Loft, LLC its teachers, assistants and employees from any and all liability and responsibility from injury, accident, illness, legal and medical fees sustained now or in the future resulting from my participation or my child's participation in any activity or use of equipment.

I understand that The Movement Loft will provide an area for personal belongings to be held during class, however, I agree that The Movement Loft LLC is in no way responsible for the loss or damage of any of my belongings while I or my child attend class.

I acknowledge that I have read this waiver of liability form. I fully understand its terms and conditions, and understand that I am giving up my rights to pursue legal action against The Movement Loft LLC, its teachers, contractors and employees.

I acknowledge that I am signing this agreement voluntarily, and intend by my signature to be a complete and unconditional release of liability to the greatest extent allowable by law.

I acknowledge that this Release has been entered into voluntarily and I am not entitled to any compensation or royalties with respect to the use of photo, video, or audio recordings by The Movement Loft LLC and partners

The Release shall be binding upon and inure to the benefit or the parties and their respective heirs, administrators, person representatives, executors, successors and assigns.

This Release constitutes the entire agreement between the parties and supersedes any and all prior oral or written agreements or understandings between the parties concerning the subject matter of this Release. This release may not be altered, amended or modified, except by a written document signed by both parties. The terms of this Release shall be governed and construed in accordance with the laws of the State of Texas.

I hereby represent that I have carefully read and fully understand all provisions in this Release. I acknowledge that I have read, understand and consent to the terms in this Release.
