COVID 19

I hereby certify that I am not presenting signs or symptoms of the Covid-19 virus. I certify that I do not have a fever of over 99 degrees. To my knowledge, I am not currently infected with the Covid-19 virus. To my knowledge, I have not come in contact with an infected person.

LIABILITY

I understand there is an inherent risk associated with any exercise program including my voluntary participation in yoga, dance, meditation or movement classes that may result in injury.

I understand that I may receive physical assists or adjustments to enhance or correct my body posture during class by either the teacher or an assistant in class. I will alert the teacher and assistant of any injury or impairment in advance before class begins or if I do not want to receive any assists.

I acknowledge that I have either had a physical examination and /or have been given permission from my physician to participate in an exercise program or that I have decided to participate in an exercise program voluntarily and without the approval of my physician and do hereby assume all responsibility for my participation in any movement activity associated with The Movement Loft, LLC.

I, my heirs, or legal representatives, do hereby waive and release The Movement Loft, LLC its teachers, assistants and employees from any and all liability and responsibility from injury, accident, illness, legal and medical fees sustained now or in the future resulting from my participation in any activity or use of equipment.

I understand that The Movement Loft will provide an area for personal belongings to be held during class, however, I agree that The Movement Loft LLC is in no way responsible for the loss or damage of any of my belongings while I attend class.

I acknowledge that I have read this waiver of liability form. I fully understand its terms and conditions, and understand that I am giving up my rights to pursue legal action against The Movement Loft LLC, its teachers, contractors and employees.

I acknowledge that I am signing this agreement voluntarily,and intend by my signature to be a complete and unconditional release of liability to the greatest extent allowable by law.

MEDIA RELEASE

I (releasor) grant The Movement Loft, LLC (Releasee). authorization to use my image, video footage, and audio recordings for commercial and promotional use in digital and print use, including but not limited to web, social media pages, printed collateral, brochures, banners, posters and signs, postcards, publications, and advertisements.

I consent and hereby authorize The Movement Loft, LLC irrevocable authorization to use my likeness in digital, and print format; including photo, video and sound assets captured during or after a class or workshop

I waive the right to approve or review photos, videos or audio clips of my likeness used by The Movement Loft LLC.

I acknowledge that this Release has been entered into voluntarily and I am not entitled to any compensation or royalties with respect to the use of photo, video, or audio recordings by The Movement Loft LLC

The Release shall be binding upon and inure to the benefit or the parties and their respective heirs, administrators, person representatives, executors, successors and assigns.

This Release constitutes the entire agreement between the parties and supersedes any and all prior oral or written agreements or understandings between the parties concerning the subject matter of this Release. This release may not be altered, amended or modified, except by a written document signed by both parties. The terms of this Release shall be governed and construed in accordance with the laws of the State of Texas.
